Returning / Lapsed Patient Registration

If you have attended our practice before, but it has been more than 2 years since your last appointment, you will need to complete a returning patient registration form before booking.

How to Re-Register

1. Download and complete the registration form attached below.
2. Bring the completed form with you to the practice.
3. Alternatively, paper copies are available from reception during opening hours.

Once your registration has been received, a member of our team will contact you to arrange your appointment.

Please note that due to high demand, there may be a waiting period of several weeks before we are able to contact you to schedule your visit.
